Complete the Registration form below and submit electronically via the "Send Registration" button. At the Confirmation page, choose your payment method, and follow the instructions to complete payment and registration. If you are paying by check either bring your check payable to "Perusa Soccer" with you on the first day of camp or call Coach Zapata for further instructions.

By submitting this registration form, you are agreeing that you release Perusa Soccer Camp and all its agents of any liability for injury or accident that may occur at the camp. You also give consent for your child’s photograph to be taken during camp and to be used in future camp publications or on the camp website for publicity and advertisement purposes only.



No refunds will be given if cancellation is less than one week prior to the start of the camp session for which you are registered. In the event of illness, refunds will be given if presented with a doctor’s note. *If camp is rained out more than two days, camper can either come to another week of camp at no charge or those days fees will be reimbursed.
